When Pete Thornton made an unexpected and suspicious trip to the Soviet Union, MacGyver followed him. They met outside Moscow where they learned that Pete's friend, Alexander Karsov, was being held as a political prisoner in a state psychiatric hospital near Leningrad. Stepanov, a Soviet agent, attempted to arrest Karsov's daughter Marya as well, but the three of them evaded capture and traveled to the hospital with the intention of freeing Karsov.
To gain access to the hospital, MacGyver portrayed a schizophrenic patient, and he was admitted to Ward K where he met other psychiatric patients and political prisoners including Dimitri, Lopat, and Chessmaster. Meanwhile, Pete posed as Dr. Torin and made the acquaintance of the sadistic Dr. Suvarin as well as Dr. Natalya Petrovich, who did not approve of Suvarin's methods.
With the cooperation of Marya, Natalya Petrovich, and several of the other patients, Pete and MacGyver succeeded in freeing not only Alexander Karsov but many of the other patients and prisoners as well.
